I wanted to get ETC card and Hokkaido Pass, but the communication was not so smoothly gone through. However, overall, it was a pleasant business with them.
The cars are not at airport and we had to take shuttle bus for 15 mins. Waiting time for shuttle meant that total 30 mins was spent getting to counter and keys
You only need a driver's license issued by a contracting country of the 1949 Geneva Convention and an IDP. The passport doesn't need to be issued by the same country.
Although there were some language barriers, we were able to understand each other. The staff was very friendly and even gave me a ride to the JR station after I returned the car.

the free shuttle bus provided by the new chitose airport does take a while to travel to Samurai shop as its last stop in the car park, will be good if Samurai can be more explicit on describing when to get off the shuttle bus, as the driver doesn't really understand English
I have an early flight and had to return my car before 830am. I had wished there is a key box to safeguard the key. I placed the key in the car instead.
